Using an IS form with inputs to create a PDF for client and me, while inputting all info

I have an uploaded PDF doc that i want realtors to fill out when sending me their referrals online, I would like this form to have inputs for their client they are referring, update the realtors information on file and also be able to have this form saved & emailed for realtor and me, as well as stored in the realtors contact, tags created so there’s a follow up call with the new client as well, card send out to the referring agent (via ZenDirect)…

who can help me with this? or other suggestions in where all info is automatically updated, so there’s no human error or handwriting required.
