Adding a Note to an Internal Form

Hi! I am creating an internal form to move customers through the follow-up process. This is replacing our current campaign that uses tasks, and now we are moving to tags. I want my staff to be able to complete the form, but type in the notes from the contact made with the customer on the internal form. I know I can add a note within the campaign, but I need them to be customized with the details from the conversation.

Create custom fields “Note Title” (text field) and “Note Body” (text area field)
Put those on the Internal form
Right after the Internal Form is submitted, in the following sequence, set up a Note process and feed those custom fields into the Title and Body of the note. That will allow you to enter your note details on the Internal Form and also have those details create a Note on the contact record.
